OnPlan Health Privacy and Security Policy

OnPlan Health is a service provider to hospitals and healthcare professionals (“Provider”). OnPlan Health allows patients to manage payment plans, and personal account information.
OnPlan Health is committed to maintaining the confidentiality, integrity, and security of any and all personal information. This Privacy and Security Policy (the “Policy”) explains how OnPlan Health uses that information in connection with OnPlan Health services through our site. “Personally Identifiable Information” for purposes of this Policy means information provided to us that identifies you, such as your name, address, phone number, email address, any account information or other information you store.
OnPlan Health privacy standards are designed to guard against identity theft, to protect sensitive data, and provide security for your Personally Identifiable Information. OnPlan Health regularly re-evaluates privacy and security policies and adapts them as necessary to deal with new challenges.
Your Private Information is not for sale

Technologies used to optimize your experience
Demographic data also may be collected at our site through the use of third-party tools. We may review the actions of the visitors of our site on an aggregate basis to determine customers’ preferences. We also may conduct research on our customers’ demographics, and interests based on the information collected in connection with use of our Website. Notwithstanding anything to the contrary in this Policy, OnPlan Health reserves the right (and you hereby authorize OnPlan Health) to share and disclose your Private Information when OnPlan Health determines, in its sole discretion, that the disclosure of such information is necessary or appropriate:
- to prevent prohibited or illegal activities; or
- When required by any applicable law, rule regulation, court order, warrant, subpoena or other legal process.
In such an event, if permitted by law, OnPlan Health will make reasonable efforts to notify you and give you the opportunity to seek a protective order.

OnPlan Health reserves the right to modify this policy at any time, so please review it frequently. If OnPlan Health makes material changes to this policy, OnPlan Health will notify you here, by email, by means of a notice to the OnPlan Health home page We encourage you to periodically review this page for the latest information on our privacy practices. The date last revised appears at the top of the policy.
